Undergraduate Student Government Complaint Form

This form was created and published at the end of Fall 2022 semester by branch leadership and our Justice & Equity leadership to be able to anonymously (or personally) report any concerns about the behavior and conduct of individuals within our organization. 

These efforts were made in hopes that members and people within the Hawkeye community will feel comfortable speaking out about issues within our organization as we continuously work to make USG a safe space for everyone. Leadership will work quickly to address any and all concerns that come through the form.

If you have instances you'd like to report please use the form below. Responses will be sent to USG staff advisors and the Chief Justice to ensure that the best parties may respond quickly and appropriately.
